Filing a Claim

All claims for damages against the City of Florence must be filed with the Florence City Clerk’s Office.

To file a damage claim, complete and submit the City’s claim form, which is available from the City Clerk’s Office or may be downloaded and printed from the City’s website. A handwritten or typed, notarized claim affidavit is also acceptable.

Please address all claims to the City of Florence and include as much information as possible regarding the incident, including the date, time, location, description of the damage, description of the events leading to the damage, and any other relevant information. If you have repair bills, invoices, or estimates, please include copies with your claim documents.

Be sure to include your name, mailing address, telephone number(s), and signature on your claim.

All notices of claim must be hand-delivered or mailed to the City Clerk’s Office and received within six (6) months of the date of the incident.

Please allow the City sufficient time to review, investigate, and process your claim. After your claim is received, the original claim documents are forwarded to the City’s Insurance Manager for investigation and processing through the City Attorney’s Office. Because each claim requires coordination with the appropriate City department, the Risk Management Office, and the City Attorney’s Office, the review process may take some time to complete. You will be notified once the investigation has been completed and a determination has been made regarding your claim.
